Changing wet or soiled nappies promptly and gently keeping skin clean and dry are basic parts of nappy-rash care. A clear routine is a more useful starting point than repeatedly trying new products. Do not try to diagnose every rash at home.

## Prepare supplies before the change

Everyone caring for the baby should know where clean nappies, cleaning supplies, a disposal bag and spare clothes are kept. Arrange things so you do not need to leave your baby alone to fetch something. If you use separate home and travel kits, check both for missing supplies.

## Keep gentle care consistent

Clean with water or suitable fragrance-free, alcohol-free wipes and dry gently. Avoid fastening the nappy too tightly, talcum powder and irritating fragranced products. Ask a pharmacist or healthcare professional if you need product advice; this guide does not select a cream or prescribe a dose.

## What should the next caregiver know?

Use three lines: time of the last change, what you noticed on the skin and any professional instructions. Do not record something as observed if you did not see it. Describe the area and what changed since yesterday rather than assigning a diagnosis to the rash.

## When to ask for help

Ask a healthcare professional if the rash persists, worsens or spreads, or if your baby has a fever or seems very uncomfortable. Notes should not delay seeking care. If a photo is requested, use the health service’s secure route; there is no need to post it publicly.
